Job summary

Wirral University Teaching Hospital NHS Foundation Trust is seeking a Cancer Care Co-ordinator to provide information and support to cancer patients, helping them navigate health and social care and access the right services at the right time.

The role includes tracking colorectal patients, coordinating service delivery, and promoting best practices across the region while handling data and audit-related tasks with strong organisational skills.

Qualifications

• NVQ level 3/4 in Health or Social Care or equivalent
• GCSE qualifications with Maths and English
• Counselling qualification
• Foundation degree or diploma level or equivalent
• Basic Cancer Course

Responsibilities

• Provide cancer information and support to help patients navigate health and social care
• Coordinate RMS tracking of colorectal patients
• Organise day-to-day running of the service and respond to emails
• Ensure compliance with policies and clinical guidelines
• Work with regional coordinators to adopt best practice
• Utilise databases to maintain records and generate statistics
• Coordinate remote surveillance programme and organ ise investigations
• Present remote surveillance workshops
• Document and monitor all care coordination activities
• Demonstrate excellent organisational skills

MS Office/IT skills

Education
