University Hospitals Plymouth NHS Trust is seeking an experienced care coordinator to support patients on the suspected cancer pathways for colorectal services. This 12-month secondment offers a route to a permanent position and welcomes temporary workers who have already spent at least 3 months with the Trust.

The role focuses on providing a single point of access for 2ww referrals, supporting cancer pathways and coordinating assessments to fast-track patients into the correct clinics.

Qualifikationen

• Demonstrable health experience at AfC Band 3 or equivalent.
• Proven knowledge of cancer pathways.
• Demonstrable experience of multi-professional working.
• GCSE English Language and Mathematics (Grades A-C) or equivalent or demonstrable experience.
• NVQ Level 3 in health or administration or equivalent demonstrable experience.

Aufgaben

• Coordinate care by providing a single point of access for patients referred on the suspected cancer pathways of Colorectal pathway.
• Track patients referred on the suspected cancer pathway to improve flow.
• Provide basic telephone advice prior to diagnostic appointments and refer on or sign-post to other sources of support.
• Coordinate the necessary assessments, appointments or investigations to fast track people into the most appropriate clinic.
• Demonstrate the ability to recognise and respond appropriately when faced with an unexpected issue.
• Escalate any delays within the diagnostic pathway.
• Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
