Job description

We are looking for a confident highly motivated experienced Clinical sister or Charge Nurse to work with one of our integrated community nursing teams, you will be working alongside registered nurses and other health and social care professionals and will support staff to provide assessment, care and treatment to adults and older people living at home who have physical health needs which may be complex and long term.

This position will require you to work autonomously within a dedicated nursing team. You will be responsible for providing evidence based high quality care to patients with life limiting illnesses and support for their families.

You will also be required to demonstrate innovative clinical management and leadership skills, initiating, and mentoring the continued professional development of individual team members, and supporting the Locality Lead in the continued development of the Community Nursing Service.

Main duties of the job

To be responsible and accountable for assessing, planning, implementing and evaluating patient care, demonstrating evidence based/best practice. To adhere to Walsall healthcare NHS Trust policies, procedures and protocols in order to promote the quality of the patient experience in the community, ensuring the highest standard is received

The post holder will be responsible for providing evidence based high quality palliative and end of life care to patients with life limiting illnesses and support for their families

To hold continuing responsibility and accountability for the quality of patient care demonstrating evidence based/best practice. To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols in order to promote the quality of the patient experience in the community, ensuring the highest standard.

To provide professional support and mentoring to individual staff members and community place based teams, initiating continued professional development to help improve the care and management of palliative and end of life patients

To maintain effective channels of communication with the wider multi-professional team, to ensure co-ordinated patient care

To support implementation of the integrated care model within Walsall healthcare Trust and work in -partnership with key organisations, promoting patient care in the right place at the right time

About us

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. Job responsibilities

Management and Leadership

  • To support the community nursing service in providing high quality individualised end of life care to patients and their carers at home; in order to improve quality of life and provide a safe alternative to inpatient care, where this is appropriate and in accordance with preferred place of care.
  • To develop communication skills to ensure support for patients and families in highly emotional and distressing circumstances, when discussing sensitive and potentially distressing information.
  • Contribute to service development and co-ordinate their implementation.
  • Be responsible for the supervision of qualified/unqualified staff. Identify training needs and facilitate continued professional development through the PDR / 1:1 process.
  • Ensure effective learning experiences and opportunities to achieve learning outcomes for students through mentorship, and for registered practitioners through preceptorship, clinical supervision and the provision of learning opportunity

Care Delivery

  • To be responsible and accountable for the assessment of care needs, the development, implementation and evaluation of care, ensuring that patients receive the highest quality of care.
  • To have knowledge of and providing care to, patients and families of different cultural, religious and ethnic backgrounds
  • Demonstrate analytical and a high level of judgemental skills when interpreting and performing acute or complex patient assessments and care planning.
  • Maintain accurate and up to date nursing records, patient confidentiality and statistical records as required by the service.
  • Work with other professionals to promote multidisciplinary working, appropriate skills transference and the creation and use of care pathways.
