Job summary

GTD Healthcare Limited seeks a Clinical Practice Based Prescribing Pharmacist to join Hattersley Group Practice in Hyde. The role focuses on medicines optimisation, safe prescribing, and delivering patient-centred care across primary and urgent care settings.

37.5 hours weekly, Monday to Friday, with flexible pattern to be agreed. The successful candidate will provide medicines advice, conduct medication reviews, and support integration with community and hospital pharmacy teams to improve

Qualifications

• Mandatory registration with the General Pharmaceutical Council.
• Specialist knowledge via post-graduate diploma or equivalent.
• Independent prescriber or working towards independent prescribing qualification.
• Membership of the Royal Pharmaceutical Society (RPS).

Responsibilities

• Collaborate with clinicians and pharmacy teams to optimise medicines and patient care.
• Liaise with ICB/PCN pharmacists to ensure consistency of prescribing and medicines management.
• Lead on governance and supervision training for learners and staff.
• Identify and resolve medicines-related risk and improve service delivery within the team.
• Develop new services or modes of delivery to meet changing healthcare demands.
• Produce timely and informative reports on medicines optimisation and service performance.
