Job summary

Hetton Group Practice in the North East is seeking a Clinical Pharmacist to join a growing practice pharmacy team. This newly developed role focuses on medicines optimisation, reviewing hospital discharge summaries, reconciliation, and repeat prescribing within a supportive MDT. You will provide expert medicines advice, lead on safe prescribing, run Structured Medication Reviews, and collaborate with GPs, care homes, and secondary care. Independent prescriber status is essential.

Qualifications

• Experience as a registered pharmacist and medicines optimisation principles.
• Ability to provide evidence-based education and training to colleagues.
• Proven track record in safe prescribing and clinical governance.

Responsibilities

• Review hospital discharge summaries and reconcile medicines.
• Lead on safe and efficient repeat prescribing and medicines reviews.
• Provide expert medicines advice to staff and patients.
• Conduct Structured Medication Reviews (SMRs) for high-risk medications.
• Support prescribing audits and cost-effective prescribing initiatives.
• Collaborate with GPs, care homes, and secondary care.
