Job summary

Barnsley Place Medicines Optimisation Team is seeking a Practice Pharmacist to provide prescribing advice and lead medicines optimisation across GP practices in Barnsley. You will collaborate with GPs, nurses, and pharmacists to implement cost‑effective prescribing and ensure guideline‑compliant medication use.

The role involves medication reviews, quality improvement, and ensuring safety and appropriate monitoring for high‑risk patients in care homes and for those with chronic diseases.

Qualifications

• Registered pharmacist with GPhC registration and current practice.
• Experience in prescribing matters and medicines optimisation.
• Ability to work with multidisciplinary teams and explain complex issues simply.

Responsibilities

• Provide prescribing advice and support to the practice on all medication issues.
• Advise on implications of prescribing and admission/referral data provided by the ICB.
• Implement changes in prescribing behaviour to promote quality, rational, safe and cost‑effective prescribing.
• Promote prescribing in line with national and ICB guidelines and policies.
• Identify and advise on clinical risk linked to prescribing.
• Undertake medication reviews for patients with chronic diseases to manage polypharmacy.
