Job summary

Northolt PCN is seeking a motivated Clinical Pharmacist to join our Primary Care team. You will deliver high-quality medicines management, provide expert clinical advice to healthcare professionals, and support safe, evidence-based prescribing in a busy NHS setting.

You will work within a multidisciplinary team, perform medication reviews, optimise long-term condition management, and contribute to audits and service improvements.

Qualifications

• GPhC Independent Prescriber qualification and registration.
• Experience in medicines management and clinical pharmacy practice.
• Understanding of confidentiality and data protection in NHS contexts.

Responsibilities

• Lead medicines management and provide clinical advice within the PCN.
• Conduct long-term condition reviews and optimise prescribing.
• Support discharge medicines reconciliation and ensure safety.
• Educate practice staff and patients on medicines optimisation.
• Contribute to audits and service development.
