Job Summary
We are looking for a proactive, forward-thinking pharmacist to join our experienced pharmacy team at Brockworth Surgery.
Main duties of the job
- The post holder will work within their clinical competencies to provide specialist pharmaceutical support to the Practice's clinical team to improve care and support safe and effective prescribing.
- The role involves providing patient-facing services via face-to-face or telephone consultations, undertaking chronic disease management, repeat prescription management, and structured medication reviews.
- The post holder will be actively involved in quality improvement, audits, and managing aspects of the Quality and Outcomes Framework (QOF).

We are a teaching practice with a team comprising 4 GP Partners, 6 salaried GPs, Nursing and Pharmacy teams, and a multidisciplinary team including mental health practitioners, Care Co-ordinators, and Social Prescribers. Job Responsibilities
- Conduct clinical medication reviews for patients with long-term conditions to optimize medicines use.
- Reconcile medicines post-discharge from hospitals or intermediate care, collaborating with patients and community pharmacists to ensure appropriate medication management and reduce readmission risks.
- Participate in multidisciplinary clinical meetings as appropriate.
- Manage repeat prescription requests, resolve queries within scope, and ensure necessary monitoring tests are in place.
- Implement medication changes following MHRA alerts, product withdrawals, shortages, or other guidance.
- Maintain comprehensive records of all patient contacts using clinical templates and coding.
- Engage proactively in audits and quality improvements, implementing recommendations.
- Provide independent medication and prescribing information to patients, coordinating with other healthcare professionals as needed.
- Participate in policy review and development related to medicines management and prescribing, ensuring adherence to local guidelines and formulary.
- Identify clinical risks at practice level and recommend practices to optimize prescribing quality and resource use.
- Maintain professional competence through continuous professional development.
- Collaborate with the Clinical Team to ensure compliance with Care Quality Commission standards for safe and effective care.
Person Specification
Qualifications
- Essential: Registered with the General Pharmaceutical Council; Holder of GPhC Independent Prescribing qualification or working towards it.
- Desirable: Masters in pharmacy (MPharm); Completion of CPPE Primary Care Pathway or willingness to undertake it; Experience in Primary Care.
