We are looking to recruit one Lead Clinical Pharmacist and two Clinical Pharmacists.
We are a large, friendly, progressive, and dynamic GP practice serving approximately 24,000 patients.
We seek Clinical Pharmacists who are team players and can deliver exceptional patient care. Responsibilities include performing patient assessments, creating and implementing personalized care plans, delivering evidence-based treatments to optimize patient outcomes, maintaining accurate records, prescribing independently in accordance with clinical guidelines, conducting face-to-face and telephone medication reviews, running specialist clinics, providing expert advice on prescribing and medicines management in primary care, and supporting patients with long-term conditions through medication management.
For the Lead Clinical Pharmacist role, prior management experience is required.
Our practice is large, friendly, progressive, and dynamic, serving around 24,000 patients. We are the lead GP practice in our local PCN and have extensive representation within our CCG.
