Job summary

EPUT is recruiting a Clinical Flow Coordinator to support the Care Coordination Centre at St Margaret's Hospital, Epping. You will help manage patient flow, coordinate safe transfers of care and assist with discharge planning across West Essex, working with multidisciplinary teams and partner organisations.

The role is fixed-term until 31 March 2027, full-time or part-time options, 37.5 hours per week, with a focus on improving patient outcomes and system performance.

Qualifications

• Experience supporting a multidisciplinary team.
• Ability to draft reports, briefing packs, data and presentations.
• Experience in health or care settings.

Responsibilities

• Coordinate patient flow and transfers of care across hospital and community services.
• Maintain patient flow trackers and data to monitor demand, capacity and outcomes.
• Support discharge planning, referrals, bed management and service improvements.
