Job summary

Essex Partnership University NHS Foundation Trust is seeking a Clinical Flow Coordinator for its Care Coordination Centre at St Margaret's Hospital, Epping. The role focuses on improving patient flow, supporting safe transfers of care and discharge planning, with responsibilities across West Essex and partner organisations.

You will maintain trackers, support MDT meetings, and generate reports to inform operational decisions, working in a fast-paced environment to deliver timely patient care.

Qualifications

• Experience in a health or care context and coordination roles.
• Ability to produce briefing packs, reports and presentations.
• Demonstrated capability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Coordinate patient flow and safe transfers of care across the system.
• Maintain patient trackers and data to monitor demand, capacity, and outcomes.
• Support discharge planning, referral screening, and bed management processes.
• Produce reports and briefing materials for operational decision making.
• Collaborate with community, acute and social care partners to improve patient care.
