Job summary

Hampshire County Council is seeking a Discharge Flow Coordinator to help older residents, including those living with dementia, regain independence after a hospital stay. You will coordinate journeys through Short-Term Services and support timely discharges with a multidisciplinary team.

The role focuses on building strong relationships with families, partners and care teams to deliver high-quality, person-centred outcomes in a fast-paced environment.

Job description

Join a dedicated service that helps older people, including those living with dementia, regain their independence following a hospital stay. As a Discharge Flow Coordinator, you'll play a key role in ensuring people receive the right support at the right time, working alongside a skilled multidisciplinary team to deliver positive outcomes and smooth transitions of care.

What you’ll do:
