Job summary

Surrey and Sussex ICB is seeking an AACC Practitioner to deliver CHC duties across Surrey and Sussex. The role includes clinical duty, assessments, reviews, care planning and case management with travel between sites.

You will provide expert CHC advice, support safeguarding, and work with inter-agency partners to ensure high-quality patient care. A degree and UK registration are essential; Band 6, 12-month fixed-term contract.

Qualifications

• Degree level in relevant subject or equivalent experience in a specialist area.
• Current UK professional registration (e.g. NMC).
• Experience in health and care environments (acute, community, mental health, local authority).
• Experience in multi-disciplinary teams.
• Comprehensive knowledge of NHS Continuing Healthcare and NHS Funded Nursing Care guidance.
• Ability to analyse complex clinical information and develop care plans.

Responsibilities

• Deliver end-to-end AACC services including CHC assessments and reviews.
• Manage enquiries/referrals from patients, public and professionals.
• Support audits, safeguarding, and inter-agency partnership working.
• Provide specialist CHC advice to professionals, patients and families.
• Ensure high standards of patient care and comply with NHS policies.
