Job summary

NHS Surrey and Sussex ICB is recruiting a Fixed Term Practitioner (Band 6) to join the AACC Team. The role covers NHS Continuing Healthcare activities, including assessments, reviews, care planning and case management, with travel across Surrey and Sussex as required.

You will work to high standards, ensuring safeguarding and patient-centered care, and maintain professional registration. The post emphasises collaboration with multidisciplinary colleagues and adherence to NHS values.

Qualifications

• Educated to degree level in a relevant subject or equivalent experience.
• Comprehensive knowledge of NHS Continuing Healthcare and related frameworks.

Responsibilities

• Deliver CHC assessments and reviews as part of the AACC service.
• Support audits, retrospective assessments, and safeguarding processes.
• Maintain up-to-date knowledge of legislation and NHS policies.
