Job summary

Frimley Health NHS Foundation Trust is seeking a Band 6 Bank Clinical Practitioner to support children, young people and adults with complex needs across the Thames Valley. The role focuses on NHS Continuing Healthcare assessments and family-centred decision making within a flexible bank workforce.

You will coordinate CHC assessments, collaborate with families and multidisciplinary teams, and work across the CHC pathway including Fast Track and PHBs, adapting to changing needs while promoting

Qualifications

• Educated to degree level in relevant subject or equivalent with up-to-date registration with the regulatory body (NMC).
• Proven ability to work in health/social care settings and to apply policies.

Responsibilities

• Coordinate CHC assessments and reviews for adults and children.
• Support pathway delivery including Fast Track, care planning, PHBs.
• Chair and facilitate MDT meetings.
• Synthesize clinical information for eligibility recommendations.
• Ensure safety, appropriateness and responsiveness of care.
