Job summary

Responsibilities

• Coordinate NHS Continuing Healthcare (CHC) assessments and reviews for adults and children.
• Support Fast Track, care planning, clinical commissioning, case management and Personal Health Budgets (PHBs).
• Chair and facilitate multidisciplinary meetings, advocating for individuals and coordinating with health and social care partners.
• Synthesize information to present robust, defensible eligibility recommendations and ensure safety and quality.
