Job summary

NHS Surrey And Sussex Integrated Care Board is seeking an AACC Practitioner to deliver Continuing Healthcare services across Surrey and Sussex, collaborating with local health and care organisations.

You will conduct CHC assessments, coordinate care plans, reviews, and case management, while contributing to audits and safeguarding. A region-wide travel requirement and commitment to person-centred care are essential.

Qualifications

• Educated to degree level with up-to-date registration with the regulatory body (e.g. NMC).
• Experience in health or care environment (acute, community, mental health, local authority).
• Knowledge of NHS Continuing Healthcare and Funded Nursing Care.

Responsibilities

• Deliver CHC assessments and reviews with standard operating procedures.
• Monitor quality assurance of care delivery and support safeguarding.
• Provide specialist advice on Continuing Healthcare to professionals, patients and families.
