Job summary

Town and Country Housing is seeking a dedicated Caseworker to support customers through the Disabled Facilities Grant and home adaptations process, guiding them from enquiry to completion. You will become the key point of contact for customers, contractors, local authorities and partner agencies, ensuring each case is managed efficiently, professionally and with compassion.

You will visit customers at home to assess needs, coordinate grants and loans, and advise on funding options and welfare

Qualifications

• Experience coordinating home adaptations and grant processes.
• Ability to visit customers in their homes and assess needs.
• Strong documentation and data recording skills.
• Ability to liaise with contractors and local authorities.

Responsibilities

• Managing a caseload of customers requiring home adaptations and support services.
• Visiting customers in their homes to assess needs and provide guidance throughout the grant process.
• Coordinating Disabled Facilities Grants, loans and private adaptation works from start to finish.
• Acting as the main point of contact between customers, contractors, colleagues and external partners.
• Completing and processing grant and loan applications and associated documentation.
• Maintaining accurate customer records, reports and performance data.
• Advising customers on funding options and welfare benefits available to support their needs.
• Liaising with partner organisations regarding safeguarding, wellbeing and support requirements.
• Supporting the delivery of a high-quality, customer-focused service that promotes independent living.
