Job summary

Active Care Group is seeking an experienced Case Manager to cover Cambridge and surrounding areas. You will independently assess clients with complex needs, develop case management plans, and coordinate services with multidisciplinary teams. Regular travel is required with some remote work options.

The role offers flexibility with three days per week minimum, full-time preferred, and opportunities to work within a supportive clinical team. A UK driving licence is essential.

Responsibilities

• Undertake detailed assessments and identify rehabilitation needs.
• Develop and review individualized case management plans.
• Coordinate rehabilitation services across sectors.
• Lead multidisciplinary teams and monitor intervention effectiveness.
• Conduct risk assessments and create care packages.
• Liaise with clients, families, solicitors and Deputies.
• Prepare assessment and review reports with accurate records.
• Support housing, equipment, education and community needs.
