Job summary

AJ Case Management is seeking a Case Manager to join a team providing case management services to children and adults with complex disabilities. Travel is required and remote/flexible working is available.

The role reports to the Clinical Lead and Business Director, with multiple areas of cover across Oxfordshire, Hampshire and London. You will assess client needs, set intervention goals, and deliver regular updates.

Qualifications

• Relevant professional qualification in nursing, social work, occupational therapy, speech & language therapy or physiotherapy.
• Current registration with the appropriate professional body (e.g. SWE, HCPC, NMC).
• BABICM registration (advanced BABICM preferable).
• Minimum 2 years case management experience.
• Experience with brain injury, neurological and complex conditions.

Responsibilities

• Provide case management services to clients (children and/or adults) with complex disabilities.
• Undertake needs assessments and set goals with clients and families.
• Produce regular reports in line with AJ Case Management standards.
• Ensure client well-being and safety at all times.
• Support clients with appointments, specialist equipment, housing and care needs.
• Assist with accessible and suitable holiday plans and costs.
• Manage a mixed and varied caseload according to skill set.
