Job Title: Band 6 Community Nurse - East Hull

We are seeking a dedicated Band 6 Case Manager within the East Hull Nursing and Conditions Team. This is a part-time position, 30 hours per week.

Key Responsibilities

1. Provide holistic community nursing care to patients with Long Term Conditions, complex wound care, and palliative needs.
2. Deliver care in patients' homes, residential care settings, and designated treatment rooms.
3. Work collaboratively within a multidisciplinary team, liaising with local healthcare providers.

Candidate Requirements

• Excellent communication skills and an adaptable approach to respond to the dynamic environment of community nursing.
• Proficiency in IT and the ability to travel between locations for work, training, and meetings. Daily access to transport is essential.
• Strong knowledge of community nursing practices.
