Active Care Group is seeking a Complex Care Specialist Support Worker to cover regional locations. You’ll support safe hospital discharges and deliver high-quality, person-centred care in clients’ homes, enabling independence and confidence.

The role involves mentoring colleagues and collaborating with Care Managers and clinical teams. Ideal candidates will have a full UK driving license, experience with complex care needs, and a calm, professional approach suitable for varied assignments across

As a Complex Care Specialist Support Worker, you’ll be part of a highly skilled team providing urgent support wherever it’s needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

• Level 2 Health & Social Care qualification (or above)
