Job summary

Crystal Care Collection is seeking a Care Plan Manager to lead care planning across multiple homes in the South of England. You will work with General Managers and Care Managers to ensure person-centred, compliant care plans and robust audit processes.

This hybrid role requires two years in care management, driving licence, and willingness to travel. You will deliver PCS training, support quality improvement, and provide reports to senior leadership to raise care standards.

Qualifications

• Two years' experience in a care management role.
• Experience with regulatory compliance and quality assurance.
• Knowledge of person-centred care planning and documentation.
• Strong analytical and attention-to-detail abilities.
• Full UK driving licence and willingness to travel across the South of England.
• Experience using PCS or similar electronic care planning systems.

Responsibilities

• Support General Managers and Care Managers to ensure robust, personalised and regularly reviewed care plans.
• Coordinate and oversee the electronic care planning process, ensuring records are accurate and up to date.
• Audit care plans and documentation, identifying trends and opportunities for improvement.
• Provide guidance to care teams on effective record keeping and appropriate care planning terminology.
• Produce quality and compliance reports for senior leadership, highlighting risks and recommending improvements.
• Support homes where non-compliance or quality concerns have been identified, implementing action plans to drive improvement.
• Deliver bespoke care planning and PCS training to new and existing team members.
• Coach and support new Care Managers in their responsibilities for care planning and PCS governance.
