Job summary

The Practice is seeking a Care Navigator to serve as the frontline contact for patients, coordinating appointments and inquiries with a welcoming, professional manner. You will signpost to GPs, nurses, pharmacists, mental health practitioners, social prescribers, and local services, while supporting day-to-day administrative tasks using clinical systems.

• First Point of Contact: Welcome patients and visitors in person, via telephone, or through digital/online consultation platforms with a professional, calm, and welcoming demeanor.
• Active Signposting & Triage Support: Conduct non-clinical assessments of patient needs to direct them efficiently to the right resource (such as GPs, practice nurses, clinical pharmacists, mental health practitioners, social prescribers, or local community services).
• Appointment & Workflow Management: Book, amend, and cancel appointments, handle prescription requests, and manage day-to-day administrative tasks using clinical systems.
• Communication & Collaboration: Act as a central communication link between patients, the multidisciplinary clinical team, hospitals, and external health agencies.
