Job description

The Integrated Care Partnership is a large GP practice and a standalone Primary Care Network (PCN). With a patient population of 32,000, the practice is ideally positioned to benefit from its unique status as a single Practice PCN.

Main duties of the job
  • Assist doctors, clinicians, and management in providing high-quality services
  • Greet and direct patients courteously and efficiently upon arrival
  • Process and signpost patients to appropriate healthcare professionals based on their presenting conditions
  • Answer general enquiries, explain processes, and respond to requests
  • Handle telephone calls, transfer calls, and manage callers' requests appropriately
  • Manage patient requests for appointments
  • Record requests for home visits and messages for doctors and nurses
  • Process repeat prescription requests following procedures
  • Provide test results to patients
  • Enter patient data into the clinical system (EMIS) and code appropriately
  • Initiate contact with and respond to requests from patients, team members, and external agencies
  • Provide cover for colleagues during absences

The Integrated Care Partnership operates across four sites: The Old Cottage Hospital, Epsom; Fitznells Manor Surgery, Ewell Village; Cox Lane Surgery, near Chessington; and Stoneleigh Medical Centre, Stoneleigh.

Job responsibilities
  • Assist clinicians in delivering high-quality care
  • Greet and direct patients efficiently
  • Respond to electronic requests and general enquiries
  • Handle incoming calls and manage communication appropriately
  • Record home visits and messages for clinicians
  • Coordinate with hospitals and other organizations for patient information and results
  • Communicate test results to patients
  • Arrange transportation for hospital appointments
  • Process computerised prescriptions as requested
  • Register new patients (permanent and temporary)
  • Enter and update patient data in the clinical system
  • Follow up with patients as requested by clinicians
  • Book appointments and perform administrative tasks like photocopying and emailing
  • Participate actively in the practice team and attend relevant meetings
  • Provide cover for colleagues during absences
Person Specification
Experience
  • Good IT skills
  • Strong organisational skills
  • Ability to multitask and meet deadlines
  • Knowledge of local health services and providers
