Job summary

Debenham Group Practice is seeking a Care Navigator to join our patient-facing team at Debenham. You will help patients access information and the right services, using askmyGP to guide clinical requests and supporting the duty GP as part of a friendly navigation team.

Experience in admin or customer-facing roles is valued; ability to work across sites and with NHS systems is essential. Shifts include Wed, Thu and Fri, with a 07:30–18:30 window.

Responsibilities

• Care Navigators are often the first voice or face a patient encounters and help ensure they get the right information and place.
• Use askmyGP as the primary workflow tool to ensure requests are recorded and acted on promptly.
• Support the duty GP as part of the wider Care Navigation team and signpost to local services when appropriate.
• Contribute to day-to-day practice administration including reception duties and managing records.
• Work across sites as needed in a friendly team.
