The Place Care is seeking an experienced Care Manager to oversee domiciliary, supported living, and complex care services in London. You will lead care delivery, supervise staff, maintain documentation, and ensure compliance with CQC standards and company policies.

You will engage with families and professionals, review care plans, and promote person-centred, safe, and well-led care across packages.

We are seeking an experienced, organised, and compassionate Care Manager to support the effective day-to-day management of our care services. The successful candidate will oversee care delivery, support staff supervision, manage care packages, maintain high-quality documentation, and ensure services are delivered safely, professionally, and in line with CQC standards and company policies.

This role is suitable for someone with strong experience in domiciliary care, supported living, complex care, or adult social care who is confident in leading staff, communicating with families and professionals, and maintaining safe and responsive care.

Responsibilities

The Care Manager will:

  • Oversee the day-to-day delivery of care and support across assigned care packages.
  • Ensure care is delivered in line with individual care plans, risk assessments, outcomes, preferences, and agreed support hours.
  • Complete and review care plans, risk assessments, client review forms, support plans, and associated documentation.
  • Monitor service quality through spot checks, audits, care reviews, observations, and feedback.
  • Support staff supervision, appraisals, performance management, training compliance, and competency monitoring.
  • Ensure staff follow company policies, safeguarding procedures, medication guidance, infection control procedures, and professional boundaries.
  • Respond promptly to concerns, incidents, complaints, safeguarding issues, changes in needs, missed calls, staffing issues, or service risks.
  • Liaise professionally with service users, families, advocates, social workers, commissioners, GPs, nurses, therapists, and other professionals.
  • Support safe rota planning, continuity of care, staff allocation, and cover arrangements.
  • Review daily notes, handover records, MAR/eMAR records, incident reports, body maps, and monitoring charts where required.
  • Ensure documentation is accurate, factual, timely, and audit-ready.
  • Support the Registered Manager with CQC compliance, governance, quality assurance, and service improvement.
  • Promote dignity, independence, safeguarding, confidentiality, equality, diversity, and person-centred care at all times.
Requirements & Qualifications
  • Previous experience in a care management, senior care, care coordination, team leader, deputy manager, or similar role.
  • Good knowledge of domiciliary care, supported living, complex care, or adult social care.
  • Strong understanding of CQC expectations, safeguarding, person-centred care, MCA, confidentiality, infection control, risk management, and care documentation.
  • Ability to complete and review care plans, risk assessments, audits, reviews, and reports.
  • Excellent communication and interpersonal skills.
  • Strong leadership, organisation, problem-solving, and decision-making skills.
  • Ability to manage competing priorities and respond calmly to urgent situations.
  • Good IT skills and ability to use electronic care planning systems.
  • Reliable, professional, compassionate, and accountable approach.
  • Right to work in the UK.
  • Level 3 or above in Health and Social Care.
  • Level 5 Diploma in Leadership and Management for Adult Care, or willingness to work towards it.
  • Experience managing complex care packages.
  • Experience supervising care staff and completing spot checks.
  • Knowledge of medication audits, incident reviews, safeguarding records, and quality assurance processes.
  • Experience working within a CQC-regulated service.
