Service Care is recruiting a Personal Advisor – Leaving Care Team on a 2-week contract to support care leavers transitioning to independent living. You will manage a diverse caseload, build strong relationships, and develop outcome-focused pathway plans with multi-agency partners.

The role requires experience with young people or vulnerable adults, strong communication, and independent work within a team. Competitive hourly pay and training allowances are provided.

Qualifications

• Experience working with young people, care leavers, or vulnerable adults.
• Ability to manage complex caseloads, assessments and plans.
• Excellent communication and relationship-building skills.

Responsabilités

• Managing a complex and diverse caseload of care leavers.
• Building positive, trusting relationships with young people.
• Conducting comprehensive needs assessments.
• Developing SMART, outcome-driven pathway plans.
• Supporting young people through their transition into adulthood.
• Collaborating with partner agencies and professionals.
• Encouraging young people to participate in decision-making processes.
• Producing accurate reports and case records.
