The role requires DBS clearance, a full UK driving licence, strong assessment and reporting skills, and the ability to work independently

Qualifications

  • Experience with Care Leavers, Looked After Children, or vulnerable youth.
  • Understanding safeguarding and corporate parenting duties.
  • Skilled in conducting assessments and creating person-centered support plans.
  • Ability to collaborate with multiple agencies and professionals.
  • Experience building positive relationships with diverse young people.

Responsibilities

  • Oversee a diverse and complex caseload of care leavers.
  • Collaborate with young people to set and achieve personal goals.
  • Conduct needs assessments and create individualized support plans.
  • Develop and review Pathway Plans in line with statutory guidelines.
  • Ensure Pathway Plans are SMART and outcome-focused.
  • Assist young people in their transition to adulthood.
  • Coordinate care planning with partner agencies and professionals.
  • Encourage positive outcomes in housing, education, employment and health.
  • Build strong relationships with care leavers and their networks.
  • Empower care leavers to engage in decision-making and service development.
  • Work independently with young people and families when needed.
  • Prepare clear, accurate reports and maintain case records.
  • Utilize supervision and team discussions for decision-making.
  • Uphold professional boundaries and pursue ongoing professional development.

Hours: Full Time

Working Hours:

  • Monday to Thursday: 8:45 am – 5:00 pm

DBS: Enhanced DBS with Adults' & Children's Barred List Required

Driving Requirement: Full UK Driving Licence Required

Requirements

Requirements:

  • Experience working with Care Leavers, Looked After Children, or vulnerable youth.
  • Proven ability to manage a diverse and complex caseload.
  • Understanding of safeguarding and corporate parenting duties.
  • Skilled in conducting assessments and creating person‑centered support plans.
  • Ability to collaborate effectively with various agencies and professionals.
  • Experience fostering positive relationships with diverse young people.
  • Strong communication, report‑writing, and organizational abilities.
  • Capability to work independently and manage workload efficiently.
  • Full UK Driving Licence and access to a vehicle.
  • Experience within a Leaving Care Service.
  • Familiarity with legislation and statutory guidance for Care Leavers.
  • Experience supporting asylum‑seeking youth and those with immigration challenges.
  • Experience working with young people with disabilities and additional needs.
