Job summary

Service Care is seeking a Leaving Care Personal Advisor in Rochdale to support care leavers aged 16+. This contract role focuses on developing and implementing individual Pathway Plans, building independence skills, and helping with education, employment and housing.

You will coordinate services across health, education and housing providers, advocate for young people, maintain accurate records, and respond promptly to safeguarding concerns, while encouraging active participation in planning.

Contract Length: 2 weeks (with potential for ongoing support)

Hours: 37 hours per week
