Service Care is recruiting for a Personal Advisor to join our Leaving Care Team. You will manage a diverse caseload of care leavers, offering tailored guidance and practical support to help them move towards independent living.

The role requires an Enhanced DBS (Adults and Children) and a full UK driving licence, with a 12-week initial contract and 37 hours per week. Salary is paid per hour under PAYE or Umbrella.

Qualifications

• Strong relationship-building and communication skills.
• Experience managing caseloads and supporting individual needs.
• Ability to complete assessments and develop support plans.
• Confidence working independently and with multi-agency professionals.
• Commitment to safeguarding and achieving positive outcomes.
• Full UK driving licence and access to suitable transport.
• Enhanced DBS covering the Adults’ and Children’s Barred Lists.

Responsabilités

• Manage a diverse caseload of care leavers with varying needs.
• Complete needs assessments and develop individualised support plans.
• Prepare clear, SMART and outcome-focused pathway plans in line with statutory guidance.
• Support young people to develop independence and successfully transition into adulthood.
• Work collaboratively with social care professionals, partner agencies, families and other support networks.
• Build positive, trusting relationships that empower young people to participate in decisions about their futures.
• Produce clear, concise reports and maintain accurate records.
• Recognise and respond to individual needs, promoting equality, inclusion and positive outcomes.
