Civic Recruitment Limited is recruiting a Personal Advisor to work within the Leaving Care Service in Peterborough. You will support care leavers with housing, education, employment and life skills, providing guidance and practical help to help them transition to adulthood.

This is a 3-month contract with a local authority. You will manage a caseload, assess needs, develop pathway plans and work with multi‑agency teams to safeguard and promote welfare.

Qualifications

• Experience working with Care Leavers, Young People, Children in Care or Leaving Care Services.
• Experience carrying out assessments and support planning.
• Strong report writing and record keeping skills.
• Experience working within Children\'s Social Care or Early Help services.

Responsabilités

• Manage a caseload of young people leaving care.
• Assess needs and deliver tailored support and intervention plans.
• Develop, review and update Pathway Plans.
• Support young people with housing, education, employment and training opportunities.
• Assist care leavers in managing finances and budgeting.
• Carry out visits to homes and other community settings.
• Maintain accurate records, case notes and reports.
• Attend reviews, meetings, case conferences and planning meetings.
• Work collaboratively with Children\'s Social Care, education, health and partner agencies.
• Promote safeguarding and the welfare of vulnerable young people.
• Support young people to develop independence and life skills.
• Ensure services are delivered in line with legislation, policies and procedures.
