Job summary

Civic Recruitment Limited is recruiting for a 2-month contract role as a Personal Advisor within a Local Authority in Peterborough. You will manage a caseload of care leavers, providing advice, guidance, advocacy and practical help to support transitions to independent adulthood and improve outcomes in accommodation, education, employment, training, health and wellbeing.

The position requires 37 hours per week with hybrid working from Sand Martin House.

Qualifications

• Experience with vulnerable children or care leavers in social care or related field.
• Ability to manage caseloads and deliver person-centred support plans.
• Experience conducting assessments, reviews and interventions.
• Proactive professional development relevant to working with young people.

Responsibilities

• Manage a caseload of care leavers adhering to statutory duties and policies.
• Develop, implement, monitor and review Pathway Plans.
• Establish relationships providing consistent advice and support.
• Assess needs and provide targeted interventions to improve outcomes.
• Collaborate with Children’s Social Care, education, health, housing and external agencies.
• Conduct home visits, assessments, reviews and welfare checks.
