Job summary

Civic Recruitment Limited is recruiting for a 9-month contract with a Birmingham local authority to support care leavers as they transition to independent adulthood. The role involves holistic, person-centred advice, guidance, and intervention while managing a caseload and delivering tailored support.

You will work 36.5 hours per week, with 2 days in the office and 3 days remote, collaborating with social workers, carers and partner agencies to ensure access to education, employment and housing.

Qualifications

• Experience working with vulnerable young people, especially care leavers.
• Experience managing a caseload and delivering structured interventions.
• Evidence of continuous professional development.
• Enhanced DBS clearance (required).

Responsibilities

• Act as a full member of the Service team and contribute to its development within the Directorate.
• Manage an individual caseload and deliver direct support to young people on a one-to-one and group basis.
• Participate in duty and referral work, responding to emerging needs appropriately.
• Develop and implement tailored support programmes to assist young people transitioning from care to independent living.
• Maintain regular contact with service users to support their development, independence, and self-esteem.
• Conduct assessments and develop personalised support plans in conjunction with young people, social workers, carers, and partner agencies.
• Ensure all young people receive their entitled services and support from relevant agencies.
• Support access to education, employment, accommodation, and benefits.
• Work collaboratively to support young people with continuing care needs and ensure a smooth transition into adulthood.
