Job summary

Conexus Healthcare CIC is recruiting a Care Coordinator to support four GP practices in Wakefield, coordinating personalised, proactive care for patients. The role includes direct patient contact, home visits, assessments and teamwork with clinicians across the PCN.

You will contribute to reviews for learning disabilities, dementia and frailty, gather information from patients and carers, and ensure timely follow-up, escalating safeguarding concerns as needed.

Qualifications

• Educated to NVQ level 3 or equivalent qualification, or proven relevant experience.
• Experience in health and social care or a customer-focused organisation desirable.
• Evidence of care coordinator training or the care certificate desirable.

Responsibilities

• Coordinate care across four GP practices to deliver personalised and proactive care.
• Carry out home and care home visits, assessments and care coordination.
• Identify concerns and ensure appropriate follow-up, including safeguarding.
