Job summary

Morden Plus Health Partners PCN is seeking a Care Coordinator to support four GP practices in the Morden area. The role involves coordinating information between patients and clinicians, and assisting with administrative tasks to ensure high-quality patient care.

You'll help manage video consultations, e-hub data, and MDT meetings, with training provided. Experience in Primary Care IT systems and strong communication skills are desirable.
