Job summary

Trinity Health Group is seeking a Care Coordinator to support four GP practices within the PCN, delivering personalised and proactive care to patients. Role involves home visits, care coordination and supporting clinicians across the multidisciplinary team.

The post involves working with patients with learning disabilities, dementia, frailty and other complex needs, contributing to reviews and signposting to VCSE organisations. Flexible patterns include full-time and part-time arrangements.

Qualifications

• Educated to NVQ level 3 in a relevant subject or equivalent qualification.
• Experience in a support role in health and social care or within a customer-focused organisation.
• Evidence of undertaking the relevant care coordinator training and/or the care certificate.

Responsibilities

• Support PCN team to coordinate care across four GP practices.
• Respond to changing service demands and contribute to effective delivery of PCN services.
• Carry out delegated tasks, referrals and safeguarding as required, in line with policy.

Education

NVQ level 3 in a relevant subject or equivalent qualification or proven relevant experience
