Conexus Healthcare CIC is seeking an experienced Practice Nurse to join our Wakefield community team on a fixed-term basis with potential to go permanent. You will work across PCN sites, deliver enhanced long-term condition care, and collaborate with GPs, nurses, and allied health professionals to improve patient outcomes.

The role requires flexible hours between 8am–6pm, travel across Wakefield District, and adherence to NHS standards and governance.

Competenze

• Registered General Nurse with UK registration and current practice.
• Experience in community or care home settings preferred.
• Ability to work within MDT and follow clinical governance.

Mansioni

• Provide patient-facing clinical care in community and care home settings.
• Assess patient needs, identify risks, and deliver nursing interventions.
• Support GPs and the wider primary care team to deliver high-quality care.
