Qualifications

• Minimum one year of relevant care coordination experience.
• Excellent organisational and time-management abilities.
• Strong written and verbal communication skills.
• Proficient IT skills including MS Office and email.
• Confidentiality, safeguarding, and person-centred care understanding.

Responsibilities

• Coordinate and manage staff rotas to ensure continuous care.
• Schedule visits and arrange cover for absences.
• Communicate with clients, families, staff, and management.
• Maintain accurate care schedules and records.
• Support care plan reviews and promote person-centred care.
• Respond to queries and changes in client needs.
• Assist with on-call rotations and after-hours support.
