Job summary

Operose Health is seeking a Care Coordinator at Trowbridge Surgery, Hackney, for 30 hours per week. You will provide care coordination and administrative support to patients, helping them access services and ensuring clear communication between patients and the practice.

You will work with GPs, nurses and other team members to coordinate care, arrange appointments, signpost to community services, maintain patient records and assist with general admin while upholding GDPR and information

• Willingness to support reception, telephone and administrative duties.

Responsibilities

• Coordinate care and support for patients, particularly those requiring additional support.
• Contact patients by telephone to arrange appointments, follow-ups and appropriate services.
• Work with GPs, nurses, pharmacists and other members of the practice team to support patient care.
• Signpost patients to appropriate health, social care and community services.
• Maintain accurate and up-to-date patient records.
• Make and receive telephone calls and deal with patient queries appropriately.
• Provide reception cover and support the front desk.
• Assist with general administrative duties, including filing, scanning, referrals and processing documents.
• Support the practice with PCN and practice projects and initiatives.
• Maintain confidentiality and comply with practice policies, GDPR and information governance requirements.
• Work as part of the wider practice and PCN team to improve patient access and experience.
• Undertake any other reasonable duties required by the Practice Manager.
