Job summary

Frome Medical Practice in Frome, Somerset, is seeking a Care Coordinator working 37.5 hours a week to provide high‑level administrative support to clinical teams with a focus on complex care and patient welfare.

You will support patients by phone and in person, manage referrals and results, and coordinate with GPs and NPs across the care team, including safeguarding and training duties as needed.

Qualifications

• Secretarial and administrative support for Clinical staff to support patients.
• Liaising with clinicians and supporting both over the phone and face to face.
• Managing and feeding back of results and referrals.
• Supporting complex care teams and safeguarding reporting.

Responsibilities

• Secretarial and administrative support for Clinical staff to support patients.
• Supporting patients both over the phone and face to face and liaising with clinicians as required.
• Feeding back of blood/radiology/urine results to patients
• Managing E Referral lists and ensuring correct pathways followed with Clinicians as required.
• Supporting GPs and NPs on Complex care team and liaison with Secondary Care
• Managing completion of paperwork and liaising with families after patient death.
• Arranging FIT Notes for patients and managing emails from Secondary Care.
• Contributing to safeguarding report requests and staff training.
• Support integration and knowledge sharing across the practice.
• Engage with patients with dementia and cancer diagnoses and carers to provide support.
• Assist in organising vaccination programmes for Housebound and Care Home Patients
• Support admin tasks related to Health Connections Team and other duties.
