Job summary

Right Care NW Ltd is seeking an experienced Care Coordinator to oversee domiciliary care services across Bury, Whitefield and Bolton. The role focuses on rostering, client assessments and maintaining high standards of care while supporting a dedicated care team.

You will work closely with the Registered Manager, care staff and families, ensuring visits are delivered on time and care plans are up to date. This office-based role offers progression opportunities within a growing service.

Qualifications

• Experience in care coordination and rostering.
• Experience carrying out client assessments and care plan reviews.
• Good understanding of care standards and safeguarding regulations.
• Excellent organisational and time management skills with the ability to prioritise.
• Strong communication skills and ability to build positive relationships with clients and teams.

Responsibilities

• Plan, create and maintain weekly care rotas to cover visits safely.
• Coordinate care visits across Bury, Whitefield and Bolton and manage routes.
• Handle daily changes to staffing and care requirements, including sickness and last-minute cover.
• Support Care Assistants with guidance, supervision and spot checks.
• Carry out initial client assessments and care plan reviews; ensure individual needs are met.
• Build positive relationships with clients, families and health professionals.
• Assist with recruitment, inductions and staff development.
• Maintain accurate care records and compliance documentation.
• Monitor and uphold quality of care and regulatory standards.
• Participate in weekend on-call rota.
