Job summary

Right Care NW Ltd is seeking an experienced Care Coordinator to support our domiciliary care service across Bury, Whitefield and Bolton.

You will manage weekly rotas, coordinate visits, assess client needs and work closely with the Registered Manager to maintain high standards of person-centred care. This full-time, permanent role offers a competitive salary from £26,000 plus on-call allowance and performance bonuses, with development opportunities.

Qualifications

• Experience in a Care Coordinator role within domiciliary care.
• Experience carrying out client assessments and care plan reviews.
• Understanding of care standards and safeguarding regulations.
• Excellent organisational and time management skills.
• Strong communication and relationship-building abilities.
• Full UK driving licence and flexibility for weekend on-call.

Responsibilities

• Plan and maintain weekly care rotas using OnePlan.
• Coordinate care visits across Bury, Whitefield and Bolton.
• Manage daily changes to staffing and care requirements.
• Support and supervise Care Assistants; conduct spot checks.
• Conduct initial client assessments and care plan reviews.
• Build relationships with clients, families and healthcare professionals.
• Assist with recruitment and staff inductions; maintain records.
• Monitor quality of care and ensure regulatory compliance.
• Participate in weekend on-call rota.
