Job summary

Choices Homecare, part of Optimo Care Group, is seeking a proactive Care Coordinator to support safe, effective care delivery across Bolton and Bury. The role blends field-based quality monitoring with office-based rota coordination, ensuring care plans, risk assessments and standards are met.

The post is full-time, field-based (3 days) and branch-based (2 days), with on-call duties and a salary of £28,000 per annum. Driving licence required; full UK work permission essential.

Qualifications

• Experience in homecare coordination or scheduling.

Responsibilities

• Conduct spot checks, observations and welfare visits.
• Complete carer supervisions and competency assessments.
• Provide coaching and problem-solving support to care staff.
• Monitor care quality and gather feedback from service users and families.
• Escalate safeguarding concerns.
• Maintain rotas and match carers to service users.
• Respond to sickness and rota changes promptly.
• Update digital systems to reflect service delivery.
• Act as key contact for carers, service users and professionals.
