Right Care NW Ltd is seeking an experienced Care Coordinator to support domiciliary care services in Bury, Whitefield and Bolton. The role focuses on organising rosters, coordinating visits and maintaining high-quality client care.

You will manage scheduling within a defined area, build strong team relationships and ensure consistent service delivery with empathy and professionalism. On-call duties are included as part of the weekend rota.

Formación

• Previous experience in a Care Coordinator role within domiciliary care, including scheduling and rostering.
• Experience carrying out client assessments and care plan reviews.
• Good understanding of care standards, safeguarding and relevant regulations.
• Excellent organisational and time management skills, with the ability to manage competing priorities.
• Strong communication skills and the ability to build positive working relationships.

Responsabilidades

• Plan, create and maintain weekly care rotas using OnePlan, ensuring all client visits are covered safely and efficiently.
• Coordinate care visits across Bury, Whitefield and Bolton, planning routes to minimise travel and ensure timely delivery.
• Manage daily changes to staffing and care requirements, including sickness and last-minute cover.
• Support the Care team with guidance, supervision, performance monitoring and spot checks.
• Carry out initial client assessments and care plan reviews to meet individual needs.
• Build positive relationships with clients, families and healthcare professionals.
• Assist with recruitment, inductions and training in collaboration with the Registered Manager.
• Maintain accurate care records, schedules and compliance documentation.
• Monitor quality of care and ensure regulatory standards are met.
• Participate in the weekend on-call rota.
