Care Coordinator - Domiciliary Care

Salary: From £26,000+ per annum, negotiable depending on experience, plus on-call allowance and performance bonuses

Location: Whitefield, Bury - Office based

Hours: Full-time

Contract: Permanent

Are you an experienced Care Coordinator looking for a role where you can make a real difference, feel valued and develop your career?

Right Care NW Ltd is looking for a warm, friendly and caring Care Coordinator to join our growing team, supporting the delivery of high-quality home care services across Bury, Whitefield and Bolton.

This is a fantastic opportunity for someone who enjoys building relationships, supporting care teams and ensuring people receive the care they need, when they need it.

Whether you are an experienced Care Coordinator who loves what you do or someone with ambitions to progress into a Branch Manager role, we are committed to supporting your development.

Role overview

As Care Coordinator, you will play a key role in the smooth day-to-day running of our domiciliary care services, working closely with the Registered Manager, care staff, clients and their families.

You will take responsibility for a manageable and established schedule of care hours within a defined geographical area, allowing you to build strong relationships with your care team and provide a consistent, reliable service.

This is a varied and rewarding position where your organisational skills, attention to detail and ability to think on your feet will make a genuine difference. A patient, empathetic and proactive approach will be essential in ensuring both clients and care staff feel valued and supported.

Duties include:
  • Plan, create and maintain weekly care rotas using OnePlan, ensuring all client visits are covered safely and efficiently.
  • Coordinate care visits across Bury, Whitefield and Bolton, planning routes to minimise unnecessary travel and ensure care is delivered at the agreed times.
  • Manage daily changes to staffing and care requirements, including sickness, emergency admissions and last-minute cover.
  • Manage and support a dedicated team of Care Assistants, providing guidance, supervision, performance monitoring and regular spot checks.
  • Carry out initial client assessments, care plan reviews and introductory visits to ensure individual care needs are met.
  • Build positive relationships with clients, families, care staff and healthcare professionals.
  • Support the Registered Manager with recruitment, staff inductions, training and ongoing development.
  • Maintain accurate care records, schedules and compliance documentation.
  • Monitor the quality of care delivered, ensuring company policies and regulatory standards are maintained.
  • Participate in the weekend on-call rota.
The ideal candidate

We are looking for someone who genuinely cares about delivering excellent service and has the confidence and experience to coordinate care effectively. You will be naturally kind, patient and approachable, with the empathy to understand individual needs and a proactive attitude towards supporting both clients and colleagues.

Essential requirements:
  • Previous experience in a Care Coordinator role within domiciliary care, including scheduling and rostering.
  • Experience carrying out client assessments and care plan reviews.
  • A good understanding of care standards, safeguarding and relevant regulations.
  • Excellent organisational and time management skills, with the ability to manage competing priorities.
  • Strong communication skills and the ability to build positive working relationships.
  • A calm, practical approach to problem-solving, particularly when dealing with last-minute changes.
  • Good administrative and computer skills.
  • A full UK driving licence.
  • Flexibility to participate in the weekend on-call rota.
Desirable:
  • Level 3 qualification in Health and Social Care, or equivalent.
  • Experience using OnePlan or similar digital care planning and rostering systems.
  • Previous experience supervising or supporting a team of care staff.

Above all, we are looking for someone who is warm, dependable and passionate about providing person-centred care, treating everyone with kindness, patience and respect.

What We Offer
  • Competitive salary starting from £26,000 per annum, negotiable depending on experience.
  • Additional earnings through on-call allowances and performance-related bonuses.
  • Annual salary progression opportunities linked to achievable performance, retention and growth targets.
  • Career development opportunities, including potential progression into a Branch Manager role.
  • Manageable workload within a defined geographical area, allowing you to focus on delivering quality care.
