Libertatem Healthcare Group is seeking a proactive Care Coordinator to manage tailored care schedules and ensure continuity of service across the UK. You will support workers, lead meetings, and liaise with clients, families, HR and Clinical Leads to resolve issues while maintaining high standards of care.

Experience in domiciliary care and strong communication are essential. Own transport is required; 25 days annual leave plus bank holidays are offered as part of a comprehensive benefits

Qualifications

• Proven experience in domiciliary care or a similar setting.
• Excellent customer service and communication skills.
• Strong logistical planning and problem-solving abilities.
• Calm under pressure, with a collaborative mindset.
• Experience mentoring or coaching team members.
• Own transport is essential.

Responsabilités

• Develop and manage care schedules tailored to individual needs.
• Ensure continuity of care during staff absences.
• Maintain strong communication with clients and families.
• Lead team meetings to foster collaboration and problem-solving.
• Recruit and match support workers to clients.
• Work with HR and Clinical Leads to resolve staff issues.
• Support ongoing training and development.
• Liaise with internal and external stakeholders.
• Supervise care and medication standards.
• Maintain accurate records and report on client progress.
• Plan and deploy resources across care packages.
• Assist with scheduling, billing, payroll, and on-call duties.
