Job summary

Solaria Group, in partnership with a national healthcare provider, is seeking a Care Coordinator for domiciliary care in Widnes.

You will coordinate daily care, manage staff rotas, liaise with service users and families, review care plans, and support CQC compliance. Monday to Friday 9am to 5pm with on-call duties.

Qualifications

• Experience within domiciliary care or care coordination.
• Understanding of care plans, staff rotas and CQC requirements.
• Strong organisational, communication and problem-solving skills.
• Ability to manage competing priorities in a busy care environment.
• Full UK driving licence and access to your own vehicle (essential).
• NVQ Level 3 in Health and Social Care is desirable but not essential.

Solaria Group is working in partnership with an established national healthcare provider to recruit Care Coordinators for its domiciliary care services in Widnes.

The Role

As a Care Coordinator, you will play a key role in ensuring the effective delivery of high-quality, person-centered care within the community.
