Job summary

DIAMOND HEALTH CARE SOLUTIONS LTD is seeking a Care Coordinator to join our new domiciliary care team in Coventry. The role requires at least 1 year in domiciliary care, with responsibilities spanning scheduling visits, client reviews, and developing care plans in collaboration with health professionals.

You will support the Registered Manager in delivery of safe, effective, and person-centered care. A driving licence and willingness to work two weekend shifts per month are essential, with

Qualifications

Minimum 1 year of relevant domiciliary care experience.
Hold a L3/L4 Health and Social Care qualification or be working towards L4/L5.
Evidence-based practice skills and knowledge desirable.

Responsibilities

Support the Registered Manager to ensure safe, effective delivery of care services.
Assist with interviewing and selecting new carers and coordinate with multidisciplinary teams.
Schedule care visits and attend client reviews; update care plans and risk assessments.
Develop care plans and risk assessments during initial client meetings.
Maintain client records, support office filing and ensure high client satisfaction.
