Hartwig Care Ltd is seeking a dedicated Care Coordinator at its Weybridge branch to support clients and guide care staff. The role is full-time (37.5 hours) with a salary of £26,650 per year, and requires a full UK driving licence with access to a car.

You will ensure timely visits, coordinate staff, and maintain clear communication across teams. Applicants should be compassionate, proactive, and highly organised, with excellent communication skills and attention to detail.

Responsabilités

• Ensure care worker visits reflect commitment to continuity and flexibility.
• Assign visits to suitable care workers according to clients' needs.
• Provide care workers with up-to-date client visit information and company policies.
• Receive and record information from care workers in a timely manner.
• Coordinate care worker review and team meetings as scheduled.
• Cross-check visits against plans to ensure accuracy and support service monitoring.
• Assist Care Managers in sharing daily updates and key information with staff.
• Answer calls and messages and route communications appropriately.

Key Responsibilities

• Ensure that care worker visits reflect Hartwig Care's commitment to continuity and flexibility.
• Ensure all visits take place on time and are assigned to the most suitable care workers according to clients' needs.
• Provide care workers with all necessary and up‑to‑date information regarding client visits and company policies.
• Receive and record information from care workers—verbally and in writing—within agreed timelines.
• Coordinate care worker review meetings and team meetings, ensuring they take place as scheduled.
• Cross‑check actual visits against planned schedules to ensure accuracy.Support improvements in service monitoring, quality assurance, and client safety.
• Assist Care Managers in sharing daily updates, visit changes, and key information with care staff.
• Answer phone calls and text messages, take messages, and redirect communications appropriately.
